Do they ever accept responsibility?

In the United States, President Bush called Berlusconi from Air Force One “to express his regret about the incident,” White House Press Secretary Scott McClellan said Friday.
The State Department released a statement saying, “We express our deep condolences for this tragic event.”
Do they ever accept responsibility? US troops shot and injured Giuliana Sgrena, the freed journalist in Iraq, and killed her guard. Nowhere can I find a US statement saying “We’re sorry and accept responsibility.” In fact, a search for her name on the U.S. Sec. of State site shows nothing so they apparently can’t be bothered to post their statement on their official website.
